• 6 photocopies of any valid ID
• 1 original Barangay Clearance (for TESDA training)
• 1 photocopy of Birth Certificate (or Marriage Contract for married women)
• 1 photocopy of Form 137/138 or TOR (with registrar’s signature; diploma not accepted)
• 3 pcs. 1x1 pictures (white background, collared shirt, with name tag)
